Also complete the SEPA direct debit mandate form (if you would like us to collect the membership fee directly): PDF / Word.
Please fill out both forms as completely as possible. The SEPA direct debit mandate only needs to be completed in the lower half of the page, and both forms must be signed.
Please send the signed applications/forms promptly to John Stackmann – do not hand them to the coach/trainer, as they may otherwise remain there (which would mean no insurance coverage).
Preferably, scan the documents and send them to John Stackmann by email as an attachment.
If this is not possible, you can also send them by post (address: Steinheilstr. 50, 91058 Erlangen, Germany).
